End of School Year Medication Pick up
All medication kept at school must be picked up on, or before, June 23th, the last day of school. Students are not permitted to transport medications home. Any medication remaining at school after the last day, will be properly disposed of unless prior arrangements have been made with the School Nurse. This includes all prescription and non-prescription medications, inhalers and Epinephrine pens.
Medication can be picked up during school hours.
New Health Forms and Medications for the 2026-2027 School Year
With each new school year, it is necessary to update your child’s Health Forms at school. All students with medical conditions (i.e. Asthma, Allergies, Seizures, etc.) must submit new health care plans each school year. The forms must be completed, signed by both parent and physician, and turned into the School Nurse when school resumes in September. The links below are samples of forms you can bring to your child’s provider to be completed. If your child’s provider has similar forms they prefer to use, it is acceptable for that form, as long as they contain the required information.
Epipen and/or Benadryl require an Allergy Action Plan
Asthma medications require an Asthma Action Plan
Seizure Medications require a Seizure Action Plan
Insulin and diabetic supplies require a Diabetes Management Plan
If you plan to bring medications to the clinic next year, you must complete a Medication Permission Form with each medication turned into the clinic and provide a written order from the prescribing provider. This includes all prescription and over the counter medications, excluding over the counter medications which are included in the district’s standing orders. Nurse Jill will be available at Bryn Mawr on August 31 and September 1 for medication and health form drop off.
